General Instructions for completing a Stallion Breeding Report

1. Type all requested information. Delays in processing this report may result if information is omitted.
2. Use a separate form for each stallion for each calendar year.
3. To assist you in correctly completing this report, please verify exposed mare’s registered name and number by looking at the Certificate of Registration. Both name and number must match with information submitted on this report. You should also verify that the owner of the mare as documented on the Certificate of Registration is the person who contracted this breeding.
4. Agreement below of the stallion’s owner (or authorized agent and their entries) along with MFTHBA member ID number are required for processing this report.
5. Retain a copy of this report for your files and/or future use.
6. Release of Registration: If a mare owner has met all requirements and financial obligations, you can release the MFTHBA to register any resulting foal by initialing the final box. Your initials will authorize MFTHBA to register the foal online or without your signature on a Breeder’s Certificate. This field is optional and should be completed individually by the listed mare.
7. This report must be submitted to MFTHBA and is due by January 15th of each year.
8. The report shall include all breeding dates or dates of exposure.
--a. Provide exact date(s) exposed – if more than one service is given mare, indicate the last date mare was exposed.
--b. If pasture exposed, provide date the mare was turned into the pasture and date she was taken out of pasture.
9. This report shall include all mares owned and bred by the owner of the stallion as well as mares owned by other parties.
10. When a stallion breeding report was sent in for the previous year and the stallion did not breed any mares in the current year, send in stallion breeding report stating no mares were bred. Just put 'none' in the blanks for first mare.
